“Acuity Degradation” is a term we’ve heard a lot about lately in urgent care and occupational medicine clinics. It refers to the phenomenon of referring (relatively) minor orthopedic, wound and eye injuries out to either the ED or direct to specialists instead of caring for them in the acute care facility.
Employers have long sought us out in urgent care / occmed clinics to avoid unnecessary referrals and the resultant delays in treatment and return to work.
In this webinar, Drs. Jay Kimmel and Steve Schutzer, of Upswing Health, share their orthopedic expertise to guide us on what types of orthopedic conditions are confidently managed in the primary care/acute care setting, what training, supplies and equipment are required, and what red flags prompt referral to the orthopedist.
The objectives are:
- Identify conditions/injuries that may be managed in clinic
- Identify red flags and decision points to refer out to orthopedics
- Identify supplies and equipment required to treat these conditions
- Identify resources to help you care for these patients

Jay Kimmel, M.D.’s Bio
Dr. Kimmel is the president and co-founder of Upswing Health, the country’s first virtual orthopedic clinic.
Dr. Jay Kimmel is a practicing orthopedic surgeon with Advanced Orthopedics New England. He is an attending at St. Francis Hospital. Dr. Kimmel attended Cornell University in New York and then the University of Rochester where he earned his doctorate in medicine. He completed his orthopedic residency at Columbia Presbyterian Medical Center where he trained with some of the pioneers in shoulder surgery. He then went on to do a fellowship in Sports Medicine at Temple University Center for Sports Medicine in Philadelphia where he participated in the care of the Temple athletes.
He is a fellow of the American Academy of Orthopedic Surgery and is Board-certified in Orthopedic surgery. He recently re-certified in 2013. Dr. Kimmel specializes in Sports Medicine with emphasis on shoulder and knee injuries. He is a member of the exclusive American Academy of Sports Medicine, whose members have demonstrated by their training and practice a thorough knowledge and commitment to sports medicine. Dr. Kimmel has earned the Subspecialty Certificate in Orthopedic Sports medicine given by the American Board of Orthopedic surgery.
Dr. Kimmel enjoys teaching and is actively involved in teaching in both the Department of Family Medicine residency program at St. Francis Hospital and the orthopedic residency program at the University of Connecticut. Dr. Kimmel’s excellence in teaching was recently recognized by St. Francis Hospital as he was selected as a St. Francis Faculty Fellow.
He lives with his wife and two daughters in Longmeadow, Massachusetts. In his spare time, he continues to work on his tennis game.

Dr. Steve Schutzer graduated with Honors from Union College and the University of Virginia School of Medicine. Following a Surgical Internship at the University of Rochester he served as Lieutenant in the Medical Corps of the United States Navy. After his tour of duty, Dr. Schutzer did his General Surgical training at the University of Rochester and then completed his Orthopedic Residency at the University of Connecticut. He was then a Fellow in Adult Hip and Reconstructive Surgery at the Massachusetts General Hospital after which he entered private practice in Hartford, Connecticut.
Dr. Schutzer is a Founding Member the Connecticut Joint Replacement Institute, a Center Of Excellence at Saint Francis, where he served as Medical Director between 2007 – 2021. He was Physician Executive for the Orthopedic Service Line at Trinity Health Of New England and Chair of the Department of Orthopedics at Saint Francis between 2017 and 2023. He is on the staff of Saint Francis Hospital in Hartford, Connecticut and a member of Advanced Orthopedics New England.
In 2014, Dr. Schutzer and two colleagues, Ms. Steph Kelly and Ms. Maureen Geary launched a consulting company, Novel Healthcare Solutions, whose mission is to establish effective and trusting business relationships between Physicians and Hospital partners – and then create orthopedic Centers Of Excellence.
Dr. Schutzer is also Co-Founder of Upswing Health, an orthopedic healthcare start-up whose charge is to help 10 million lives alleviate suffering from MSK pain by the end of 2023.
Dr Schutzer is also a Founding member of the Moving To Value Alliance, Inc. a grassroots non-profit organization with a mission to create a value-based healthcare ecosystem with the highest quality health outcomes at a reasonable cost for plan sponsors and their members, beneficiaries or employees.
