  • It is important to have knowledge of the population, informed leadership, and the knowledge to create health solutions to impact the health of a population, providing the right care, at the right place at the right time. Measurements Can be included in productivity, early intervention of health issues, and job duties to job demands.
  • It is important when interacting with a population to LISTEN and ask a lot of questions, the population drives the knowledge needed to develop and co-create the program and a leader to take on the transformational work. Information for counties can be found at CommunityHealthRankings.org.
  • There are many models that provide on-site programs for employers who use
  • HRAs to determine metrics, “DTE”, Di-rect To Employer. Many employers do the HR annually as baselines. Repeats should not be done before six to nine months.
  • Working with primary care doctors is important. Giving the information to the employee to share with their providers is important.
  • Marketing the concept to employers focuses on the successes of the program, improving health, improving cost, and improving the experience of care.
